Un homme de 70 ans avec une hypertension très ancienne et un contexte vasculaire (dyslipidémie, ex tabagisme) mais sans accident coronaire ou cérébro-vasculaire, consulte pour renouvellement de son ordonnance sans aucune plainte fonctionnelle.
Son traitement habituel comporte nébivolol 5 mg/j + hydrochlorothiazide 25 mg/j, atorvastatine 20 mg/j, aspirine 160 mg/j.
A l’examen clinique :
— 170 cm, 80 kg,
— PAc 140/88 mmHg, PAd 130/85 mmHg,
— ras par ailleurs, pas d’insuffisance cardiaque.

— créatininémie 100 µmol/L, DFGe? 80 ml/min/1,73m2,
— Na 136 mmol/L, K 3,4 mmol/L,
— glycémie 5,8 mmol/L, LDL cholestérol 1,20 g/L, uricémie 480 µmol/L (80 mg/L).
